Scope of Practice

Services provided through Carly Cavaliere, LMT, consist of massage therapy only.  

Massage therapy is a profession in which the practitioner applies manual techniques with the intention of positively affecting the health and well-being of the client. My services are not intended as an alternative for proper medical attention for specific conditions. I will not be working under my Physician Assistant scope of practice in your home, and any acute injuries, conditions or questions that are outside the scope of practice for massage therapy will be referred to a physician or other healthcare professional. As a Licensed Massage Therapist, I do not diagnose or treat any medical conditions. I may request clearance from a medical doctor prior to treatment if indicated based on client history.

Existing and New Medical Conditions

It is the responsibility of the client to keep the Massage Therapist informed of any medical treatment that Client is currently undergoing, any relevant medical history or any changes in health conditions, including but not limited to pregnancy, cardiac conditions and hematologic conditions. The Client may be requested to provide written permission from their physician prior to massage treatment. Please notify me of any allergies or sensitivities.

Informed Consent

Prior to your first visit, you will be asked to sign a consent form stating that you have read the professional massage therapy policies and procedures, understand them, and agree to comply. Prior to each massage session, the treatment plan will be discussed with you.

Confidentiality

I treat all Client visits and treatments as confidential. I will not discuss your visit, treatment or medical history with anybody without your permission. If it becomes necessary to discuss something with your medical doctor, I will ask for your permission first.

Respect to Client Needs and Boundaries

The Client may choose to leave on as much clothing as they’d like for comfort, may refuse any massage methods or ask therapist to stop massage at any time. I am happy to work longer on an area or move on if you request it, adjust pressure, temperature and music volume. The Client will always be modestly draped and only the area being treated will be undraped. Sexual interaction or discussion of any kind between Client and Massage Therapist is never appropriate. For the protection of the Therapist and the Client, the Therapist reserves the right to end a massage session at any time and deny a Client’s request for a return visit. In the event that a massage session is ended due to Client’s inappropriate behavior, the Client may be charged for the entire session.

Sanitation and Hygiene

All equipment and linens that come in contact with the Client will be cleansed in an appropriate method. Before and after each session, the Massage Therapist will wash hands and forearms with hot water and anti-bacterial soap. The Client should come to the session clean. The Client should inform the Massage Therapist of any breaks in the skin or areas to be avoided.

Payment of Services

Cash is very much preferred otherwise payment may be made via venmo @carlycavaliere-lmt. Gratuity is not required, however, always very much appreciated!

Cancellation & No Show Policy and Fees

Should you need to cancel or reschedule, please notify me via email or text message at least 24 hours in advance. Any cancellations with less than 24 hours of notice are subject to a cancellation fee of 100%. Clients who are not home for their scheduled appointment without giving any prior notification are subject to a no show fee of 100%. In the case of an emergency, please call to avoid being charged.

Insurance Billing

I currently do not accept insurance. You are responsible for full payment at time of service.

LIABILITY DISCLAIMER

IN NO EVENT SHALL CARLY CAVALIERE, LMT BE LIABLE FOR ANY DAMAGES OF ANY KIND, INCLUDING BUT NOT LIMITED TO, DIRECT, INDIRECT, SPECIAL, PUNITIVE, EXEMPLARY, INCIDENTAL, OR CONSEQUENTIAL DAMAGES, OR ANY DAMAGES WHATSOEVER IN EXCESS OF THE AMOUNT YOU PAID TO CARLY CAVALIERE, LMT FOR THE SESSION IN QUESTION, EVEN IF PREVIOUSLY ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. WHETHER IN AN ACTION UNDER CONTRACT, NEGLIGENCE OR ANY OTHER THEORY, ARISING OUT OF OR IN CONNECTION WITH THE THERAPIST’S WEBSITE.

By use of the therapist’s website, you agree to and shall indemnify and hold harmless, Carly Cavaliere, LMT from and against all claims, debts, liabilities, demands, obligation costs, expenses, actions, causes of action and claims for relief, including those brought by any third party, including reasonable attorney’s fees, asserted against or alleged to be caused by this website or any services provided by the therapist.
